History form
Follow up
Consultation
Behaviour 1 to 1's are on veterinary referral only, which means your vet needs to fill out the referral form that can be found here.
Once the referral details and medical history have been received, you will be sent a behaviour history form to fill out and return to Charlotte.
The introductory consultation will be arranged after all forms are received. This is theory based with a tentative diagnosis and treatment plan provided.
Follow up consultations are to support you with your treatment plan and can last up to an hour. The total number of follow up's will vary between cases.

Opening hours
Due to personal circumstances, with recently becoming a new mom, Thursday's 10am-4pm is when I will be open for consultations.
Throughout the week Monday-Friday I will be answering emails, messages, and working on behaviour plans.
Saturday and Sunday I am closed.
